Your Rights
As a patient, you have the right to:
- Be treated with dignity, courtesy, respect, and without unlawful discrimination.
- Receive information about the identity and credentials of the clinician responsible for your care.
- Receive understandable information about your condition, proposed treatment, material risks and benefits, reasonable alternatives, and expected costs before agreeing when practicable.
- Participate in decisions, ask questions, request clarification, seek a second opinion, and refuse treatment.
- Know that a prescription is based on independent clinical judgment and is not guaranteed.
- Privacy and confidentiality as required by law and described in the Notice of Privacy Practices.
- Access and request amendment of your medical record as permitted by law.
- Request reasonable communication accommodations and accessible services.
- Receive information about fees, billing, cancellation, and refund policies.
- Make a complaint or report a concern without retaliation.
- Be informed when telehealth is not appropriate and when in-person or emergency care is needed.
- Receive continuity, transition, or referral information when a program is discontinued or care must be transferred, as clinically appropriate.
Your Responsibilities
As a patient, you are responsible for:
- Providing truthful, complete, and current health, identity, location, contact, and payment information.
- Disclosing diagnoses, medications, supplements, allergies, pregnancy status, substance use, prior adverse reactions, and other information relevant to safe care.
- Asking questions when you do not understand instructions, risks, costs, or alternatives.
- Following the agreed treatment plan, completing required testing and follow-up, and notifying the clinician if you cannot or choose not to follow recommendations.
- Using medication exactly as prescribed, storing it safely, and never sharing, selling, transferring, or misusing it.
- Reporting side effects, emergency symptoms, changes in health, or care received from other providers.
- Maintaining a respectful environment and not threatening, harassing, recording without permission, or abusing staff or clinicians.
- Protecting account credentials and using secure communication channels for sensitive clinical information.
- Paying valid charges and canceling recurring services according to the disclosed terms.
- Seeking emergency or in-person care when directed and understanding that portal messages are not continuously monitored.
